Job Overview
Hospice Nursing to work within your schedule.
This is a great opportunity for a parent wanting a self-managed schedule. Or an RN wanting to add a wonderful hospice experience to their life. Or… an opportunity to move to a better hospice opportunity.
Pathway Hospice and Palliative Care does hospice differently. We are "Positive Experience First," for everyone, including our staff. We provide an enhanced experience for the patient and family, leading to great satisfaction not just for the patient and their family, but all of us who are involved. We all have better experiences. You are well supported; our Admin and DON are always working to assist and streamline your efforts. Our EMR is easy and effective.
As a Hospice Nurse, you are the primary clinical contact for the patient and family. Familiarity with hospice and demonstrated commitment to patient-centered care is essential. This position involves working in patients' homes, with their loved ones, being a guide and teacher, and helping them gain a fuller understanding. You need to be competent and confident in your clinical capacity.
Responsibilities
- Be the calm, capable, compassionate face of Pathway Hospice and Palliative Care with patients and their families. Understand that hospice is entirely different from other parts of healthcare — patients on hospice understand that medical interventions will no longer fix the problem and that we are managing symptoms. You must be able to support and align with that understanding.
- Anticipate healthcare issues; hospice patients decline, so we need to be aware and pre-emptive about changes in their condition. Emergent should not be an emergency — it should be anticipated.
- Manage complex symptom control such as chronic and acute pain, wound care, and whole-person support systems.
- Provide specialized hospice and palliative care, ensuring comfort and dignity for terminally ill patients.
- Document all patient interactions and medical records accurately using our exceptionally easy EMR/EHR system.
- Be a great teacher — educate patients and families on disease processes, medication administration, nutrition, and advance directives. Teach them to be confident and knowledgeable so they can act and make informed decisions.
- Help the family find resolutions within their reach. The more they participate and are engaged while the patient is alive, the less regret they will feel afterward.
- Healthcare decisions remain in the hands of the patient and family; it is our job to help them make informed decisions.
- Support patients with diverse needs, such as those with developmental disabilities or dementia care requirements.
- Ensure infection control protocols are followed diligently across all patient interactions.
- Maintain compliance with all healthcare standards, including NCQA standards, HIPAA, and hygiene guidelines.
Qualifications
- Active RN license with current registration in Utah.
- Clinical experience; critical care experience preferred.
- Proficiency with EMR/EHR charting.
- Ability to perform physical examinations and diagnostic evaluations confidently.
- Strong understanding of physiology, anatomy, pharmacology, and medical terminology relevant to end-of-life care.
- Excellent communication skills for effective patient assessment and family education.
- Ability to self-manage and work independently in home health visits or inpatient units.
- Certification in Basic Life Support (BLS) required; additional certifications like ACLS or PALS advantageous.
Join our team as a PRN Hospice RN to make a meaningful difference in patients' lives during their most vulnerable moments, while enjoying flexible scheduling tailored to your availability. We value compassionate caregivers committed to delivering exceptional end-of-life care through professional expertise and empathetic support.
